Under the general supervision of the Director of Social Accompaniment and Tenant Support, the incumbent receives and evaluates requests for various psychosocial interventions and ensures referral to the appropriate resources and stakeholders. This person intervenes in complex situations or crisis situations or interacts with several systems. The incumbent is responsible for submitting applications for changes of accommodation of a biopsychosocial nature where his/her expertise is required. He or she intervenes mainly in the homes of tenants and in their living environments.
-Directs tenants to the appropriate resources and makes personalized referrals in the health, social services and external partner network to meet the specific needs of tenants with psychosocial problems.
-Ensures the support and follow-up of the reference.
-Meets tenants during home visits and/or company offices.
-Assesses the special needs of tenants with psychosocial problems.
-Ensures collaboration and works in consultation with internal resources.
-Receives, evaluates, directs and follows up requests for housing changes for psychosocial reasons.
-If necessary, intervenes as a contact person in support of tenants in eviction files.
-Keeps abreast of available environment resources.
-Keeps abreast of the evolution of laws and regulation.
-Performs interventions in crisis situations.
-Bachelor's degree in social work and one year of experience in social work in crisis situations.
-Excellent knowledge of spoken and written French and English as well as technical methods and communication principles.
-Good knowledge of the social services network as well as the role of the various social stakeholders.
-Good knowledge of crisis intervention.
-Capacity to intervene with different clienteles and problems.
-Ability to adapt and resourcefulness in a context of growing population needs.
-Ability to analyze social issues.
-Be in possession at all times of a valid driver's license and not be under any suspension or driving prohibition.
-Be an active member of the Ordre des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec.
